We will give you a pregnancy statement or fill out the form your employer gives you. Our assistant will sign this statement. The statement must be submitted to the employer by 34 weeks of the pregnancy. You can contact our assistant during the consultation hours for questions or pick up the statement during consultation hours. In general, we do not (e-) mail a pregnancy statement.
